Problem

If a user sends a file to human revision from InstantTranslate, it is needed in some cases, that more meta-information is passed by the user to the PM. 

Solution

The pop-up that appears, after clicking on "send to human revision" is extended by the form fields

• Project title (pre-filled with the file name; required)
• Order number (empty by default; not required)
• Any additional custom fields, the sysadmin may have defined and configured, that they should show up in this pop-up

The contents of the form fields should also be present in the mail send to the PM who is informed about his/her new project.
