MQM tag can be deleted completely

      Problem

      MQM tags that were added to the segment thus being inside the <ins> tag can be deleted by the other user. This is an unexpected behavior because what is added by one user should become the <del> if deleted by another user.

      Solution

      Find the cause and fix the issue.
